Example 6: KC - CDC Sustains its Work by Creating an RFA Merger Proposal and Finding a Permanent Home

One tactic for sustainability of a project is to incorporate the initiative’s activities or services into another organization with a similar mission. After its multi-year grant funding ended, the Kansas City Chronic Disease Coalition, a coalition that was created to address chronic disease among ethnic minorities, innovatively created a Request for Application - Merger Proposal to solicit like-minded organizations who could adopt the Coalition and its work. This successfully led to the Coalition’s adoption by the Mattie Rhodes Center, a non-profit organization that provides bilingual and bicultural health services and supports tailored especially for Latinos.
