Annual RTR Consumer Satisfaction Survey

We welcome your feedback on how well the Reducing the Risks (RTR) Teen Pregnancy Prevention Project is doing to prevent adolescent pregnancy in Dade County. For each item, please circle the number that best shows your satisfaction with that aspect of the initiative. Results of this survey will be compiled by the Work Group on Health Promotion and Community Development at the University of Kansas, and will be used to help us determine our goals for the coming year.
