___You understand that incentives should be provided regularly and promptly at certain clearly defined milestones.
___You know some possible incentives.
___You have decided if you need and want an incentive plan.
___You have consulted with possible recipients of incentives.
___You have written a draft of your incentive program.
___You have consulted again with recipients.
___You have revised the plan as needed.
___You have implemented the plan.
___You have reviewed the program after a predetermined period of time.
___You continue to revise the plan as needed.
___You understand that consistency, follow-through, involvement, and review are necessary to sustain the incentive program.
