I manage a statewide tobacco prevention and control coalition funded by the National Cancer Institute. We have a state-level coalition and five local coalition which are funded. We also have coalitions/task forces in five communities which receive no federal funds, but are provided materials and technical assistance from my office. I am always looking for good information on strategic planning and am interested in working with groups to train them on using policy and media advocacy to promote their issue(s).

I provide training and technical assistance to collaborative, community (county) groups in Michigan.

Your community tool box is a great resource that we can use in the City of Ft. Worth. We have established a new division, Health Information Assessment Outreach Division, whose primary focus is to create a "Healthy Ft. Worth". We have six community based teams led by a community nurse (R. N.)that will have the support of 2 community health aides. The resources you provide will enhance the skills of our community based teams whick will help us in becoming a healthy city. I work in the Behavioral Intervention Research Branch (BIRB) in the Division of HIV/AIDS Prevention-Intervention, Research, and Support (DHAP-IRS) at the CDC. We have several community projects. In collaboration with 8 investigators, we are developing and evaluating a community-level HIV intervention for young men who have sex with men (YMSM). We will assess individual and community changes. In another project, the Community Indicators Project, we are trying to develop a model and measures for a community-level HIV intervention that seeks change in community characteristics that influence risk behavior and the sustainability of interventions.

I am the Executive Director of a nonprofit organization called The Common Enterprise. We were created as part of a national community building intiative of the Rockefeller Foundation. Our mission is to help build a stronger Greater San Antonio region by increasing the quality and quantity of civic and political participation and leadership, and by helping inform and improve public policy. We run various programs, including leadership programs for young people (sophomores in high school), neighborhood leaders, and elected officials. We are currently helping the City of San Antonio with a communitywide revitalization effort. Part of our charge is to collect best practices, lessons learned, tools, etc for our group to consider as we create a menu of options for San Antonio. Any help or suggestions would be greatly appreciated.

I work for the Hawaii State Dept. of Health (DOH) in the Cardiovascular Disease (CVD) Prevention & Control program. We are coordinating a pilot project in a local community with their health clinic that combines the efforts of six DOH programs in an effort to improve health status particularly with respect to CVD, Diabetes, Tobaco, Nutrition, Physical activity, and Community cohesiveness in Health. The hope is that this experience will provide the infrastructure for a model that can be transplanted to other state communities with similar health challenges.
