____You understand the advantages and disadvantages of determining service utilization.
____You have decided what sort of services you're going to survey.
____You have also decided whether to measure service coordination.
____You have compiled a basic inventory of services.
____You have decided what information you want to gather about these services.
____You have gathered data on how much the services are being used.
____You have analyzed your results.
____You have written a report of your results.
____If your report is internal, you have shared it with staff, your Board of Directors, and your funders.
____If your report describes service utilization in several organizations across the community, you have shared it with other organizations.
