Tool #1: Survey of Goals

Conducting the survey of goals will help your group develop a list of community changes to be sought. It may also be useful in your future strategic planning efforts.

The purpose of this survey is to help set priorities for_________________________ . We would like your evaluation of the importance and feasibility of each of the proposed changes.

In the first column (Importance), please circle the number that shows how important you feel the listed change is in preventing or reducing __________________________ . In the second column (Feasibility), please circle the number that shows how easy (feasible) it is to reach the objective.
