Tool # 1: Satisfaction survey

Here there is a sample of a satisfaction survey form. This kind of feedback evaluation is very useful because it allows you to find out if your program is working well, and what the volunteers have to suggest for improvements. Being an anonymous source of feedback, people will freely discuss what they like and dislike about the mentoring program they are involved with.
