Answer: Please check out the Youth Risk Behavior survey, which is from the CDC (Center for Disease Control and Prevention). You should be able to use google search to find this survey. While this specific survey is extensive, it should serve as a good guide to develop your own. Also, check out the website for the Violence Prevention Center, which is located in Colorado. This organization may have a gang related survey.
