Stumbling on your website, I would say it has almost been the best thing that has happened to me surfing! It has given me a stepwise explanation on the rudiments of setting-up an NGO. Despite its tremendous assistance, I would like to see ready-made sample questionnaires on community assessment. Thank you.

Please tell us a little bit about yourself:
I am a graduate student and a 20 year employee of Jefferson County Dept. of Health, in the Division of Community Health. At UAB School of Public Health my area of interest is Health Behavior and Community Systems Building. For years I have worked under the direction of others to deliver behaviorial-based health promotion programs to the community. Though I did not have a masters degree,I was up-to-date, well read and attended every workshop and seminar I could find about community-based interventions. Finally, at age 57, I decided to attend graduate school at UABSOPH. I wanted to connect theory to the interventions we were trying to implement at the Department of Health. For years I've wondered, what is the missing component, why is it that nothing really works for any length of time? Contrary to everything that had been said in the Department; I am learning that the missing component is "TRUE" community involvement, throughout every phase of an interventions. From Assessment - Evaluation
How have you used the Community Tool Box to support your work and make a difference?:
I will use the CTB at work as a resource in implementation of the MAPP public health assessment process and as a resource for assignments in school. Greater assessibility adds value.
