I am an RN and coordinate a Family Support program. The program identifies, tracks and monitors "at-risk" children and their families. This program is supported by our community hospital. The team consists of a Medical Director, a behaviorist, 2 RN's, 1 LSW and a half-time clerical worker. Our target population is birth to age 18yrs. or upon graduation of high school. It is a free service. We link families with community agencies, work collaboratively with the agencies and schools, and provide other services,ie: parenting classes, home visits, education and more.
