Anyway, you can see in Community Toolbox the entire Chapter 22 (/en/table-of-contents/implement/youth-mentoring), which I think may be close enough to your interests. It is focused on mentoring programs and includes examples and tools. I think, for instance, that inventory for mentors will be adequate, if adapted, also for your purposes.
