Hello,

I am working on a macro analysis project for a course in my social work program. There is a huge problem of firearms kept in the home unlocked and loaded in an area of the city I live in. My topic is unrestricted access to firearms and related violence (accidental deaths, theft of firearms, criminal misuse). I want to implement a public awareness campaign to educate members of the community on firearm safety and how to properly store their firearms. I want my intervention to be primarily based on education through mass media, but I also think starting a petition and drafting a new bill that requires firearm owners to safely store their firearms is important. I'm not sure that this component can be added into my awareness campaign though. Is it too much to plan and carry out? Should I just focus on public education and not creating systematic change?

Aloha, my name is Ginger Otto, I am a Preschool teacher at a Catholic School here on the Island of Kauai , Hawaii . I am a passionate gardener as well and we have the space to utilize just not the funding for a educational outdoor addition to our Preschool grounds. My dream is to build a walk in Chicken coup with nesting boxes along both sides so our preschoolers could daily get the eggs and they in turn would supply our cafeteria with the eggs needed for cooking in both the breakfast and lunch meals we provide for our school. It will be a very educational and enriching experience for them as well as contributing to the sustainability of our school. I have never applied for a grant and am a little intimidated about the thought but passionate enough about this goal I am willing to try to see what I can do. I guess my question is does this sound like something that may quailify for funding through Community Toolbox?? If so what are the steps I should take or where do I begin? Mahalo for your time. Hello Advisor,
My name is Frank Wieman and I live in Lorain, Ohio.
I am in the process of starting a non profit foundation with as main objective "share the power of sports for underprivileged children!". Because I believe sport can help children to improve themselves both physical as mental. They can learn life long skills. That's why I am very enthusiastic about this. I am new to the non profit sector and encountered the Community Toolbox. This looks very sophisticated and well maintained! At the other hand I have difficulties about where to start, it looks overwhelming to me. Here does my question appear.

I'm interested in making this my career--to create shared value by using the community toolbox to decide on a community solution and then apply it through a community partnership or buying a small business and applying that solution strategy to the current business strategy.

I am totally confused, as I have read so much. I cannot figure out what precisely to put in the following:
1) Itemized Description of proposed my program.
2) Itemized Description of proposed pilot study (what is this?)
3) Itemized Description of proposed evaluation of the pilot study including a tool (Six Sigma, TQM) to improve the proposed piloted process
4) Itemized Description of what other health care organizations and/or NGOs have done to address my initiative.

I am following the Promoting Healthy Living and Preventing Chronic Disease An Action Planning Guide for Communities

Can you please provide examples where I can see precisely what to put in these documents. I have gathered all the data there is. I have my goal, vision, mission statement, strategies, and action plans, which are all evidence-based, that concern:
1. physical activity,
2. healthy eating,
3. tobacco use, and
4. access to care and preventive services.

I know my 5 strategies are:
1. providing information and enhancing skills
2. modifying access, barriers, and opportunities
3. enhancing services and support
4. changing the consequences (incentives)
5. modifying policies and broader conditions

My framework for Promoting Healthy Living is the IOM (2003):
1. Assessment & Collaborative Planning
2. Targeted Action & Intervention
3. Community & System Change
4. Widespread Behavior Change
5. Improvement in Population level Outcomes

I have planned for new or modified programs, policies, and practices in the strategies. I do not know what section (e.g., description of proposed program, pilot study, evaluation of pilot study) I need to put this in.

However, I have not been able to meet with anybody in town - I have only had 8 days to look this up. I do know from talking to people on the phone that my county in Arkansas has very little active coalitions due to budget cuts and losing funding.

I am not getting a grant or implementing social marketing. My teacher knows that I have not met with anyone - I cannot give details of meetings.

My initiative deals with Pope County's community sectors of the: business community, health organizations, school district, and broader community.

My teacher does not want us to give her a detailed logic model.
Plus this can only be 5-7 pages long. APA 6th edition, doubled spaced, with citations. The reference page can be page 8+. I am totally vexed.

Can you please help. I have done all the work. I cannot find examples anywhere on line except for CHNAs that have not-for-profit hospitals doing community needs assessments. Yes, I am bringing the hospital in as a potential collaborator, but I am to act as if there is no coalitions, or action taking place. Plus, no one in the community seems to want to help with the severe problem of CVD, cancer, diabetes, tobacco use, etc....I know what are my modified risks. I know what are my key drivers. I know what are my vulnerable populations.

I just do not know what to place in what sections.

A small "public library" committee is trying to start action to that goal in Byron, MN, a rich, professional suburb of Rochester Minnesota with the Mayo Clinic. Our community started as a small 600 pop. rural farm community and has grown over the years to a bedroom community of Mayo Clinic professionals, senior citizens, and a superb flourishing elite school system. We would like to have a small library. We have received funding support of the Byron Community Foundation to develop an information sheet for distribution to Byron organizations for a later community support survey. I am looking to develop an assessment of baseline health information for a group of people in a different country that has asked for a health clinic to be built with the help of another outreach group. The outreach group needs to find a way to establish why the community wants the health clinic and what it will be used for, along with a baseline assessment. My degree is in public health so I have been asked to help with this. It has been so long since I have created something like this and was looking for a place to start as there are a number of obstacles to overcome with this assessment (like language, culture/societal norms etc). Any guidance would be appreciated.

I am working on a project that is trying to connect multiple groups, across professional sectors as well as community members, in an effort to reduce recidivism/enhance community stability for individuals and families after incarceration. One of the things we are trying to do is create a resource guide/reference guide type document for people to reference about what services exist, and basic information on how they are structured. For example, a mental health professional could reference it to get a basic idea of how community corrections works, what services they provide, how the funding works, and a general contact number/site for more information. Or a case manager in a correctional facility could reference it to see how a person quaifies for Medicaid, or how substance abuse treatment is paid for in Iowa and how the waitlists work. I've done a lot of looking around to try and find some guidance or even an example of what I am trying to do- basically a source of information to better connect people to services that already exist to try and reduce duplication, identify gaps, and reduce the "silo" effect. Not a directory, but it would have important contacts in it. Is this something you have any resources you could point me towards? Or language to describe it even? If not no worries, I might be making it more complicated than it actually is.

Hello! Do any of you provide (a 90-minute) presentations/discussions to not-for-profits on your toolbox--a summary of the modules, how it works, its efficacy in communities, etc.? I'm with a hospital assn., and our members are all NFPs. We are putting on a conference in March (16-18, in Bozeman), and want to provide some conceptual education (that gets into the how-tos) about building coalitions to improve the health of their communities. Many of them are small, rural/frontier, and this is fairly new for them. I'd like to start at the beginning with them to show how it can be done, no matter their size or location. Thank you.

Hi,
I am an ASU student in Phoenix area, working on my Doctorate of Nurse Practitioner (DNP) and I am wanting to do a project that will help people change their Lifestyle. With an increase in diseases that are lifestyle related, such as obesity, heart disease and diabetes, it would be fantastic for providers in primary care to have community program to help with this increasingly worrisome problem. If providers could send their patients for education and support, progress could be made in the community toward reducing the diseases caused by poor lifestyle habits. I would like to do something with diet as there is evidence to support eating 80 % whole plant based foods, but it is difficult to do. However with the right education and support it can be done. I am wondering if there are others in the Phoenix area interested in working on an initiative to help people with lifestyle changes. I may be asking the wrong person, but I thought it could be a start and / or you could direct me in the right direction.

Is there anything in your tool box concerning States changing to understand that it is everyone that is in essence responsible when a felon gets re-incarcerated? Do you have anything in your toolbox that explains how communities can support felons more to stay out of prison? The reason I ask is there isn't much of that kind of support going on. That is a change that needs to happen. Help with getting a job, a place to live and support to keep straight. You have in your tool box information on how to form coalitions, well maybe communities can form coalitions where business and housing personnel can work with prisons to help with making a much smoother transition for the felons.

We have a problem in our community with educational attainment. Roughly 40% of 18-24 year olds in our community do not go on to college. 35% of them have no high school diploma. What can we do to get started to implement a Degrees Matter program. I don't know where to start. At first I wrote a 15 minute speech citing statistics of education attainment and median household income compared to similar small towns doing a lot better job of encouraging education. I thought funding a better library program would be a great place to start but after speaking with the library director she said that we didn't need to make improvements as funding is already coming in that respect.

I want this community to do better than being known as the infamous prison town that it is. With a median household income of about 39000.

Hi there!

I've completed a community assessment of a neighbourhood in Toronto. Some important primary data I have observed is the numerous community and recreational services, as well as a large park that is the centre of the community. I am trying to implement a social marketing ad but I am not sure what I can do without making any assumptions about the community population (i.e. I wanted to hold a health fair that brings all the services to one place so the community can access it and see all the available resources to them, but is that assuming they don't already use these resources?). Please let me know if you have any suggestions.

The Sanniquellie Youth Association is the conglomeration of various Youth groups within Nimba County. It is an umbrella organization that in-house all small youth group and grooming them to have one voice so as to take up the challenges of our social development. I am interested in starting an organization in my town to provide community members, especially families, outreach opportunities, as well as cultural education seminars/workshops for children roughly 4-10yrs old.

I covet any suggestions, feedback, direction that could come my way regarding this endeavor. I am headed to look through your toolkits now and over the weekend will be working on firming up the mission/vision of the organization and hopefully drawing a crystal clear line showing how cultural awareness/understanding goes hand in hand with community service. Something like if people know more about each other, they feel more connected and are more willing to give...

I live in a VERY affluent community, very white, very much lacking diversity and community service and I would like to see these two things grow and thrive here and need as much help as I can get building a case, finding funding, connecting community service and cultural education, etc....

Hello! I have a "lengthy issue" for which I am seeking your advice...... but first of all, I wish to thank you for the work you do. My name is Jacqueline A. Juhl, RDH, MSDH Candidate (double emphasis: Dental Hygiene Education and Dental Public Health). As a graduate student, I learned of your website and have continued to access it for work in my rural community to help form an oral health coalition. I currently teach at downtown Seattle dental hygiene baccalaureate program (quite the commute!) and teach a three part Community Oral Health Series, among other classes. I have encouraged my students to access your site in their course research and have provided them with links to specifics from your site in my PPTs. Thank you all so much for the great work you do!
My issue/question is this: I see such a gap in equity to access to oral health care in our present health care delivery system. Also, as we continue to establish credible oral-system disease interactions, the void in inter-professional oral health knowledge and education has promulgated a lack of oral care for hospitalized patients in our country, a void not seen in the UK and other countries globally. It is now established that the lack of oral care for hospitalized patients negatively impacts health outcomes, can precipitate additional cardio-vascular events, and costs patients thousands of dollars in additional hospitalization days. Together with a small group of "Alternative Practice" dental hygienists in Washington State and with my experience in hospital dentistry, and based on current research of disease processes, I hope to contribute to improving health care outcomes for all, including hospitalized patients. In my future doctoral work, and through other efforts, I hope convince hospital CEOs, Boards, and the public of the need to staff hospitals and emergency departments with advance practice dental hygiene professionals and dental hygienists as they do any other "therapist" as is currently done elsewhere globally. Oral disease prevention through education and skills training for both other healthcare disciplines and patients is essential. Can you advise me how best to convince hospital CEOs and Boards to recognize the evidence and open their minds and doors to this change in our current care delivery system? My alternative practice colleagues and I had begun some grassroots work with individual hospital CEOs and I have begun to broach the subject with my local rural hospital CEO who was a great help during my thesis research on Non-Traumatic Dental Complaint patient Emergency Department utilization. Your work on focus groups and advocacy has inspired me to even come to you with this issue. I hope you can advise me how best I can be a more effect advocate to gather momentum for this greatly needed workforce change which would significantly improve health outcomes and prevent future suffering. I need your help desperately. I am the secretary of my neighborhood association and my leadership has abandoned me to carry it by myself. I am the last remaining board member of the largest neighborhood association in my city. The association has imploded as a result of a councilman who has manipulated board members which led to the founder of the association want to dissolve the association. City by laws do not permit politicians to use neighborhood associations for political purposes, the problem is the councilman does not care about the city charter and is only interested in advancing himself. Tomorrow I will be having an exploratory community meeting where I will create a forum like environment that will allow the people of the community to provide input about their needs and the shape of the association. My other problem is nobody wants to volunteer to be a part of the board. This too I will be addressing at the meeting tomorrow. I need your guidance on how to address all the matters shared, my goal tomorrow is to engage the community in meaningful dialogue that recognizes the importance of their participation while sending a clear message that we will remain independent of political influence. I have told the councilman that we will not be having any speakers, he informed me then he will be there absolutely be there like it or not. I look forward to your advice and guidance.
